::The address is 1250 Punchbowl St. If GPS takes you to Beretania street and you're driving, you will find that you have to go around and come down Punchbowl to make a right turn into the rather oddly designed driveway. Don't hop the curb! Somewhat limited Pre-Paid parking with a 2 hour max before you have to go to the pay station and pay again. They DO ticket. So remember to put the receipt on the dash in a visible spot. If you see one glass doorway you are in the back of the building that does not allow public entrance - walk around to the front where there are 3 separate glass doorways. The double doors are the main entrance and the security guards in there are nice. The bathrooms are located in this area. Birth, death, marriage and divorce certificates are through the tinted door to the left of the main entrance. If you're here for a marriage license (for when you're going to get married) look for the door to the right (if you're facing it) of the main lobby double doors.

::So after briefly reading all the reviews on this place, I have to say.... pre-order it online FOR PICK UP. it's your best option!! I've ordered a birth certificate online before, but I had ordered to get it mailed to me. I think that took a few weeks to a month, more or less. so this time, I ordered online as well, except I chose the "pick-up" option. so I did that yesterday and got here to pick it up. (side note: it is a metered parking lot. you do not need to type in your license plate or stall number. there is a minimum of $0.50 for half an hour, $1.00 for one hour, or maximum of $2.00 for two hours. it will print out a parking ticket for you to display "face up on drivers side dash" in your car.) I paid the $0.50 minimum parking fee and stood in the "Prepaid Pick-Up" line. just to give you an idea, this is at 13:00 (1:00 pm). the parking was almost full but had a few stalls open so there was no wait for that. there were only four people already in line for prepaid pickup. and it didn't take long at all. make sure you have a form of ID (driver's license, passport, etc) in hand ready to go. the person at the counter will ask for that for verification purposes. she was very nice. and it only took about two minutes to get my birth certificate. 👍🏼 so just order online and select the pick up option. I still had parking time left over. this was definitely the complete opposite of what I was anticipating. 😀

::Marriage license office opens at 8am compared to the main office. Got there 730am. Nice Staff let us in 745am. Took number #1. We were only ones there. No long line. Staffer went over marriage application that was completed online. Made some corrections and confirmed new married names. Took an oath then was explained to get married by a state registered officiant. Marriage certificate available 3 business days after officiant register it with state and copy available online after 24 hours. If need multiple copies, can order online. Best to order several copies to save money instead of ordering one at a time. When picking up copies, go to service window outside next to main office. if performer registers you online, your temporary certificate available online after 3 days. you will get an alert email from them. at that time you can order copies online and pay. you can pick up copies as soon as you paid online. When picking up at room 103 (vital records issuance office) take a number and wait to be called. show your ID and bring your payment confirmation in case. i was 4th person in line but went straight to grab a number. . in and out 10 minutes. got there 7:15am and line was started to form. all in all pleasant experience. staff was nice.

::Very corrupted place that DOH Hawai'i! Virginia Pressler as a Director should be investigated and removed from her position, same goes for Veronica Mitchell, Keith Ridley and Branden Nakamura. My daughter Bianca Krizek (former 90's fashion model) died in QMC and we were concerned about some narcotics given to her. After few emails, letter and phone calls they promised an investigation into my daughter's death. When I received a report from their investigation and compared it with a letter from Patient Care Services (signed by Dr Gallacher), both letter and their report were identical. 2 narcotics in question - Fentanyl and Ativan - their surveyor??? wasn't able to find in my daughter's medical record during the survey, even though I have provided them with a date/time, when those medications were given to her. At that point I was questioning the intelligence of the surveyor - maybe they should have sent a monkey to conduct the survey. But later on I confronted them with a copy of their report and the pages from my daughter's medical record, where those medications are clearly documented in 2-3 different places as administered. They remained silent, no response to my email. When I called them and wanted to speak to Veronica Mitchell I was told by Branden Nakamura, that she is outside the country. Really??? I guess I would be too, if I would stage an investigation into someone's death. Survey into my daughter's death was staged, meaning never happened. Informations in their report to me were given to them by Dr Gallacher from QMC. I was lied to and this is how public is being deceived by the people, whose supposed to work for us. The most corrupted agency, obviously they don't care about the public. They're so ignorant, it almost seems that they're not even aware of the fact that their salary is generated from our tax money. My daughter was profiled right in Emergency room, admitted to ICU - unnecessarily, given narcotics she had no need for, used by a residents to practice on, her carotid artery was perforated, she suffered anoxic brain injury, multiple infections she didn't come in with and that's all while she was in the care of healthcare professionals??? in QMC. And all this in the name of illegal organs/tissues harvesting. QMC harvested from my daughter without a consent. When I requested a consent from the hospital for Legacy of Life referral - this was a last order entry into my daughter's medical record, they ignored me. Of course, how do you produce NONEXISTENT consent. DOH Hawai'i by covering up and lying for QMC, this gives everyone the impression that DOH Hawai'i has their own part in illegal organs/tissues harvesting, which is currently going on in Queens Medical Center, Honolulu, Hawai'i. These is a warning to all the people of Hawaii, especially vulnerable ones to avoid this hospital or if you really need to go there, always have someone with you from the very beginning.

::Experience was a little painful to get a copy of birth certificate since I did not initially grab a number ticket. Mandatory to apply online. If applying in person, must request on their tablet from their office. Recommend paying by credit card if able to. Then you can utilize the faster prepaid line. Parking police really looks for cars without paid parking receipts. It only costs a dollar per hour. I saw two people frustrated about getting parking violation tickets.
