  I would not recommend putting a loved one at this facility especially if they have any type of communication disabilities or need full care with standing, dressing eating what ever the case unless you are able to visit daily. I visit daily yet there are still so many issues that are not addressed. My opinion is the staff is not informed or trained on how to properly listen and take time with the residents needs. If you have questions or concerns you will not get an answer due to them passing the issue to someone else. Not all but most of the staff (mostly the aide's) act like its to much trouble to take time with a resident who can not communicate. They are seriously under staffed. My opinion is this facility has so many issues that I believe the State needs to go in unannounced, not once but several times a month. If I was able I would have my mom moved to another facility, and that's not off the table. I will continue to monitor her until I can find another place for her. She absolutely hates living there and they hate to see me coming to visit daily

  I opened a gmail account ONLY because I wanted to do a review that I feel needs to be done. If I wasn't so distraught at the time of my mother's passing, I would have had an autopsy done on her before she was cremated, because I feel that this place abused her. In fact, when she passed away, she passed away with a bruised forehead that a local and great hospital had no explanation for. I have pics to prove it :( They said that there was no reason why she should have had a bruise on her, that they don't just appear. This place MAY be good for someone who is awaiting medicaid and is COHERENT! My mother had dementia and I believe without a shadow of doubt that they abused her. She became aggressive (just blocking her mouth) when it came time for her to take her meds (something I've never seen before, but did witness). I believe that's because she was frustrated and knew what they were doing to her but couldn't relay it with us due to dementia. My mom was a gentle, gentle soul. I promised her that I would get her out...once I got her into a new facility, she was never aggressive, took all meds, and ate her food...3 days later, she passed away. DO YOUR HOMEWORK! DO NOT PUT YOUR LOVED ONE HERE! RIP Mom! I did a review for you!

  Concord Healthcare Group, the company that owns Trinity does not pay their bills. Any vendor providing service or contemplating doing business with this company should beware of this issue.

  Stay far away. Employees checks always bounce, so they don't even care about fheir job. Bad bad company. Don't even think of working there or letting your family stay there. ALL BAD!!!

  I volunteer at Trinity three times a week and I read, sing and talk to the residents. Almost every time I come in, the residents are playing bingo, watching a movie or sometimes a lot of them are gone because they're at the movie theater or in Wal-Mart. It makes me happy that a lot of the residents here do not complain about their families not visiting or about never getting to go out of the facility because they are always occupied having fun or doing other activities enjoying their time here. This is a permanent home to most of them and the staff are their family. They are caring, loving and dependable. I volunteer at other nursing facilities during the week but I always enjoy coming to Trinity the most, the residents are more jolly here and they tell the best stories! This is my honest opinion and it comes from what I've seen and experienced through the last year I've been coming.
