My first time in a Skilled Nursing Facility at Avamere of Beaverton. After surgery, I spent 4 days there to recover after my skiing accident and I was pleasantly surprised at how organized this place is and how upbeat a lot of people are! They have their ducks in a row. Mia the director always greets you with a warm hello, LadyByrd came by to chat to make me feel welcomed, Barbara - getting everything that everyone needs at the speed of light, Melony and Robert - making sure everyone has their meds and offering to bring me coffee :b , Este - who gave me a shower after not having one for over a month! Rica who is always going above and beyond with a great attitude to extend a helping hand. Spanish mama who gave me extra protein sausages when I needed it =). PT, Brice for his kindness and wealth of knowledge and Dr. Julien for making it happen to discharge me =). And to all the CNA and nurses that helped me 32A, you have been wonderful and the cooks that have cooked all the delicious meals.Thank you all!

I had my 93 year old brother at Avamere for about 3 weeks. His cognition is excellent but he lives alone and had 2 falls that injured muscle and soft tissue. The Staff seemed fairly good. He had a private room and the meals seemed to be good. However, when he needed something and pressed the nurse call button, there was seldom a punctual response. On one occasion, it took 40 minutes for someone to come to his room and the room was less than 20 feet from the nurse’s station. If he had been having a heart attack or stroke, his chances of staying alive would have been slim. Staff also tends to gather in the halls and laugh and talk loudly; not what people who are in pain want to hear.

Avamere Rehab of Beaverton, OR My 93 year old mother was sent to Avamere Rehab of Beaverton, OR from Legacy Good Samaritan hospital following surgery from falling and breaking her hip. It was the closest “desirable” rehab facility to their home in Scappoose, OR. Forcing my 93 year old father to drive over 40 minutes each way to visit his wife. If this was the most desirable rehab facility I’d hate to see what the others are like. While the care appeared to be okay, it was like pulling teeth to get any detailed information from any of the staff. We had to basically figure out everything on our own. I repeatedly told them that my mother had dementia and that she would forget anything they told her in 30 seconds. Also, that my father is hard of hearing and doesn’t always catch everything. So please write everything down and put in her room or email important details to us. Of course that never happened. We were never given the “welcome” packet as they said we should have and promised to provide multiple times but we never received it. Simple questions were: What are meal times? Can we bring her food? What clothing should we bring for her? How is laundry handled? What are the hours for her physical therapy? What are the doctors names and contact info? What are visiting hours? This basic info should have been readily available without having to beg for it. After repeated requests and angry emails we finally received some info, but progress reports were never sent although we asked for them to be provided twice a week. It took way too long for the nurses to respond to call lights. She was supposed to have physical therapy one day at 11:55 and there was a big sign on her door to make sure she had her medication 30 minutes prior to PT. My father and I were there and no meds still by 11:40. I had to track down the meds person and see that she received her pain meds immediately. If we had not been there not sure she would have even gotten them. When I pressed them on getting the doctors contact info in an email they said they were not sure they could provide that. WHAT?! That’s absurd! Ultimately, I found it with a simple internet search and I was able to talk to the doctor directly. I have never heard of a medical facility not providing an attending physicians contact info to a patient’s direct family members who were authorized to receive that info under HIPAA laws. Also, the parking there is abysmal, not enough paved parking requiring parking on uneven gravel/dirt surfaces. And, the sidewalks into the facility are broken and uneven. Both of these result in a serious trip hazard for visitors, most of which are elderly. I see a lawsuit coming if someone falls like my father almost did. The only good thing my father had to say about it was he tasted some of her meals than thought the food was good. I don’t expect perfection, but when you ask a simple question you should be able to get an answer without having to beat someone over the head for it. COME ON AVAMERE you can do a better job than this since this is a “for profit” facility. Make some simple changes.

Avamere in Beaverton Oregon is the best. I give them a higher rating than 5 stars. The staff is so loving and caring! I can’t thank them enough for all that they have done for me, making my stay much better than I could ever imagine. The staff at Avamere Beaverton cares about you as a person more then just a job! Once again I can’t thank them enough for all their loving care! Tonia the administrator is the best!

My mom is here now recovering from a massive stroke. She is a young, vibrant, amazing woman. So for her to suddenly need so much care has definitely been a big shock and incredibly difficult time for her and our family. But thankfully, she came to Avamere Beaverton. We love their staff and care team! And so does my mom. Nurse Tania, Juan, Hailey, Hannah, Sam, Mona, Megan, Debbie, Elizabeth, Dr. Chiruse- those are just a few of the amazing people taking care of her right now. We really appreciate them and my mom has made significant progress under their skilled care. It isn't easy sending a loved one to a SNF during a pandemic, in fact it feels absolutely terrifying. If you're researching SNFs right now, like I was just a little over a month ago, I want to let you know it's going to be ok and Avamere Beaverton is a great place. They are all one floor, with windows in every room. If you want to visit your loved one at their window, you can. We do all the time. It's tough not being able to go inside, but I appreciate how careful their team is and I'm really impressed with their Covid-19 safety screening and testing procedures. When you go to visit, you'll see how nice the facility is. They decorated the lobby really cute for Halloween and even did a fun, Nightmare Before Christmas theme. If your loved one likes bird-watching, this is a great place for them. You'll see bird feeders by almost every window! They have a really cute courtyard too. My mom tells me everyday how sweet the staff are and how much she appreciates them. As her daughter, I'm constantly worried and wondering how she is doing, especially since I can't go inside. I promise you, this team really cares. They appreciate proactive communication and you should never hesitate to reach out and be a part of your loved one's health advocacy. Anytime I was worried about anything, I was always able to chat with someone who helped me and made sure my mom was ok. I'm so glad we chose Avamere Beaverton, they've made a big difference in my mom's recovery and for that we are forever grateful.
